Each type of CSCS card has specific requirements and therefore applications must include certain types of evidence before a card can be issued.
Failure to provide the appropriate evidence will result in delays to your application being approved.
Evidence required according to CSCS card type
When applying, please upload a copy of a completion certificate for one of the following:
- RQF Level 1 Award in Health and Safety in a Construction Environment.
- SCQF Level 4 Award in Health and Safety in a Construction Environment.
- SCQF Level 5 REHIS Elementary Health and Safety Certificate.
- NOCN/CSkills Awards Construction Health and Safety (F/618/0738) unit.
- A CSCS approved alternative qualification.
When applying, please upload the following:
A completed Enrolment Evidence Form confirming that you are registered onto an apprenticeship, including which qualification(s) with full titles and pathways you are registered for.

1. Letter or email from your Training Provider or Awarding Organisation confirming registration onto a relevant construction related NVQ or SVQ which includes:
- Your Name
- Qualification level and title of the NVQ or SVQ you are registered to
- Date of registration (must be within the last 2 years

If you hold membership with a Professional Body please upload one of the following:
- Membership certificate.
- Membership card (both sides).
- Receipt of payment.
- Screenshot from your Professional Body’s online portal/directory.
Which includes:
- Your name.
- Professional body name.
- Your membership level.
- Your membership number.
- The date, showing that your membership is for the current calendar year.

When applying, please upload the following:
A completed Enrolment Evidence Form confirming that you are registered onto a qualification or training programme (such as T-Levels or Trainee Skills Development Programmes) requiring a minimum of 30 days’ work placement.
When applying, please upload one of the following:

- A completion certificate, along with any accompanying Certificates of Unit Credit for a construction related NVQ Level 2 or SVQ Level 2.
- A City and Guilds of London Institute Craft Certificate, along with apprenticeship completion papers if you have any.
- CSCS Approved Apprenticeship Standard completion certificate, along with your NVQ/SVQ or End Point Assessment completion certificate (where applicable).
When applying, please upload one of the following:
- A completion certificate, along with any accompanying Certificates of Unit Credit for a construction related NVQ level 3 or SVQ level 3.
- City and Guilds of London Institute Advanced Craft Certificate, along with any apprenticeship completion papers if you have any.
- Approved indentured apprenticeship completion certificate (e.g.NJCBI, BATJIC, SBATC).
- CSCS Approved Apprenticeship Standard completion certificate along with your NVQ/SVQ or End Point Assessment completion certificate (where applicable).

- A letter or email from your professional body.
- A screenshot from your professional body’s online portal/directory.
- A receipt of payment for membership.
Membership evidence supplied must include the professional body name, your membership level, your membership number and must show that your membership is for the current calendar year.
This card is for people who have not held a CSCS card before and are working through probationary periods while employers assess their suitability for employment. This card lasts for six months and is not renewable. There is no requirement to upload supporting evidence for this card type.
